Am I required to pay a copay when I’m seen at the office?
Most insurance plans require patients to pay a co-payment or deductible. These payments are due at the time of your visit. The amount depends on your contract with your insurance company. We accept cash, personal checks, and credit cards (Discover, Mastercard, and Visa).
What financial information do I need to bring to the office?
Always bring your current insurance card. You will be required to complete a patient information form. (Patient Information Form)
Do I need a referral?
It depends on your insurance contract. If your contract requires primary care physician referrals or prior authorizations make sure these are obtained before your scheduled appointment.
Will my insurance pay for a screening colonoscopy?
Using the information on the Procedure Financial Form, contact your insurance company to verify if you have coverage for screening or routine preventative care services.

What insurances do you accept?
Consultants in Gastroenterology participates with most major insurance plans including Medicare. Contact our billing office at 440 461-2793 with questions.
What if I don’t have insurance?
CIG accepts credit card payments and offers uninsured discounts,
financial hardship discounts (for those who qualify), and
payment plan options. Please call our billing office at
440 461-2793 and ask to speak with our Financial Counselor
for assistance.

How do I get my medical records transferred to CIG?
You will need to complete a Medical Records Release form. To print a form, click on the office link below for where you want your records to go.
If you want a form faxed or mailed to you, please call our office at 440-461-2550 to request a form.
Once the form is complete, you can fax it back to us or give it to your current healthcare provider.

I am moving out-of-state. How do I get a copy of my medical records?
To receive a copy of your records, we need a signed authorization from you and an address where you would like the records sent. To print a form, click on the office link below for the location where you were seen.
If you want a form faxed or mailed to you, please call our office at 440-461-2550 to request a form.
Once the form is complete, you can fax it back to us or give it to your current healthcare provider. Your records will be mailed or ready for pick up within one week.

Medication
How do I get a refill on my medication?
To receive a refill, please call when you have at least one week of your medication left. Call 216 461-2550 and select your physician’s office number. You will be connected with their secretary who can assist you.
Can I receive a supply of sample medications?
Sample medications are used so the doctor can determine if the medication will work for you before you fill a prescription. Unfortunately we receive limited amounts of sample medications so we are unable to provide more than a trial supply to each of our patients.

Procedures
I misplaced the preparation instructions for my procedure. Who should I call?
Call 216 461-2550 and select your physician’s office number. You will be connected with their secretary who can assist you.
If I need a procedure, how long does it take to get an appointment?
Because most procedures are performed in our own ASC, we have greater flexibility with our schedules. Most procedures can be scheduled within one week of your office visit.
When will I find out the time of my appointment?
A nurse will call you the day before your procedure to give you the time of your procedure.
What if I need to cancel my procedure appointment?
If you need to cancel your appointment, please call 216 461-2550 and select your physician’s office number. You will be connected with their secretary who can assist you. If you call outside of our regular business hours, please leave a message with the answering service. Cancellations made later than 48 hours before your appointment may incur a cancellation fee.
Do I really need someone else to drive me home after my procedure?
You must have someone drive you home after your endoscopy procedure. The medications you will receive have sedative effects that slow your reflexes so it is unsafe to drive after your procedure.
How long will it take to get the results from my procedure?
When you are in the recovery area after your procedure, the doctor will tell you what he found. If a biopsy was taken at the time of your procedure, results are generally available within 10 days. You will receive a call when the results are received by the office.
